Perioperative hypothermia is a frequent clinical complication resulting from the cold environment of the operating room and prolonged skin exposure, leading to adverse outcomes and increased healthcare burdens. To address this issue, this narrative review discusses in detail the currently common warming strategies for perioperative hypothermia .Forced air warming (FAW) systems are widely recognized as the most effective intervention for maintaining core body temperature. Additionally, alternative technologies, such as circulating-water mattresses, carbon-fiber resistive heating systems, self-regulated heated air garments, self-heating blankets, and chemical heat packs, offer diverse advantages and disadvantages. Passive warming methods, including thermal reflective blankets and cotton blankets, provide a cost-effective solution, albeit with reduced efficacy compared to active warming measures. Recent advancements have focused on improving both active and passive warming approaches to balance effectiveness and cost-efficiency. While FAW remains the gold standard, other systems offer specific benefits, such as improved portability and reduced costs, making them suitable for use in diverse clinical scenarios. Effective perioperative temperature management reduces hypothermia-related complications, decreases healthcare expenditures, and provides substantial social and organizational benefits. Thus, selecting the most appropriate warming intervention in clinical practice requires a tailored approach, considering both patient-specific needs and resource availability.

Ultrathin reflective foils (URFs) are widely used to protect patients from heat loss, but there is no clear evidence that they are effective. We review the physics of thermal insulation by URFs and discuss their clinical applications. A conventional view is that the high reflectivity of the metallic side of the URF is responsible for thermal protection. In most circumstances, the heat radiated from a well-clothed body is minimal and the reflecting properties of a URF are relatively insignificant. The reflection of radiant heat can be impaired by condensation and freezing of the moisture on the inner surface and by a tight fit of the URF against the outermost layer of insulation. The protection by thermal insulating materials depends mostly on the ability to trap air and increases with the number of covering layers. A URF as a single layer may be useful in low wind conditions and moderate ambient temperature, but in cold and windy conditions a URF probably best serves as a waterproof outer covering. When a URF is used to protect against hypothermia in a wilderness emergency, it does not matter whether the gold or silver side is facing outward.

A product which has been heavily promoted for a number of years is an aluminized plastic foil designed to reflect 80% of body heat. However, a large body of experience points to the following: 1. A minimal amount of heat is radiated by a clothed body. This can easily be demonstrated with a thermocamera. 2. The plastic foils is impermeable to both water and water vapour. Thus condensation is formed on the blanket's interior. This virtually eliminates the reflective properties of the aluminum foil, often after only 15-20 minutes. 3. The blanket usually works best when the wind velocity is less than 9 m/s, otherwise the wind tears it into pieces. The greater the wind velocity and amount of clothing worn, the less the effect of the blanket. 4. Water accumulation on the clothing inside the blanket rapidly conducts heat from the skin. The blanket and the aluminum then take over and conduct heat 9,300 times faster than air. 5. It is supposed to reflect radar waves. Theoretically yes, but practical tests made in lifeboats at sea have showed that it gives no help. 6. The plastic film is inflammable.
