Attention deficit hyperactivity disorder (ADHD) is a common neurodevelopmental disorder modulated by genetic and environmental factors. Approximately 20-55% of individuals with ADHD experience concurrent sleep disturbances, yet the dynamic relationship between the two and their shared genetic mechanisms remain unclear. This study analyzed 11,288 participants (52.1% male) from the Adolescent Brain Cognitive Development Study using the Sleep Disturbance Scale for Children. We found a robust positive correlation between ADHD and sleep traits, though ADHD medication weakened this relationship. Latent growth curve modeling revealed that ADHD symptoms significantly impacted sleep traits with a lag effect. Polygenic risk score analyses showed significant but very small associations (explained variance ~0.28-0.30%). Pleiotropic analyses identified two loci associated with ADHD and excessive somnolence (implicating 30 genes), and cross-cohort comparisons with UK Biobank insomnia data uncovered 12 additional pleiotropic loci (98 genes) enriched in brain tissue. Functional enrichment analysis highlighted excitatory and inhibitory neuronal subtypes in visual and frontal cortices, suggesting roles in synaptic transmission and neural circuit development. Protein-protein interaction network analysis identified 10 hub genes involved in cell growth regulation and neurodevelopmental processes. This study demonstrates robust ADHD-sleep associations, with medication moderating this link, and provides preliminary insights into their shared genetic architecture.
Sleep disturbances are significantly prevalent among adults with Attention Deficit Hyperactivity Disorder (ADHD), yet limited qualitative research has explored the subjective experiences of this population. This qualitative study aimed to investigate the relationship between sleep problems and ADHD in adults, focusing on their lived experiences and adaptive strategies. Using interpretative phenomenological analysis (IPA), semi-structured interviews were conducted with 9 adults diagnosed with ADHD who reported sleep difficulties. Data were analyzed through thematic analysis to identify patterns in participants' experiences. Three main themes emerged: (1) The vicious cycle of sleep disruption and ADHD symptoms, (2) Emotional and cognitive impacts of sleep-ADHD interaction, and (3) Individual coping strategies and adaptation mechanisms. Findings revealed that sleep problems exacerbate ADHD symptoms, creating a complex interplay that affects daily functioning, emotional regulation, and quality of life. Participants developed various adaptive strategies including environmental modifications, routine establishment, and self-medication behaviors. The study provides insights into the subjective experiences of adults with ADHD-related sleep problems and highlights the need for integrated treatment approaches addressing both conditions simultaneously.
Objective: The primary objective of this study was to evaluate the prevalence of multiple types of sleep problems in young adolescents with ADHD. Method: Adolescents comprehensively diagnosed with ADHD (N = 262) and their caregivers completed well-validated measures of sleep problems and daytime sleepiness. Participants also completed measures related to medication use, comorbidities, and other factors that could predict sleep problems. Results: Daytime sleepiness was by far the most common sleep problem, with 37% of adolescents meeting the clinical threshold according to parent report and 42% according to adolescent report. In contrast, prevalence rates for specific nighttime sleep problems ranged from 1.5% to 7.6%. Time spent in bed, bedtime resistance, ADHD inattentive symptoms, and Sluggish Cognitive Tempo (SCT) symptoms were significant in the final model predicting daytime sleepiness. Conclusion: Adolescents with ADHD commonly experience problems with daytime sleepiness that may significantly affect their functioning, but this may not be directly attributable to specific sleep problems.
