Scientific journals trade in the currency of knowledge and ideas. JGIM’s commitment to readers (and therefore our expectation of authors) is that material published in the journal is original. But what does it mean for a piece of work to be “original?” Certainly all scientific writing has an intellectual heritage; even Newton stood on the shoulders of giants. Yet even in the best of circumstances, the provenance of an idea may not be readily discernable from the reference list. And circumstances are not always ideal. Recently, it came to the attention of JGIM’s editors that a manuscript just published online in another journal (Article B) bore a clear resemblance to a manuscript published in JGIM approximately six months earlier (Article A). On closer inspection, there was reason for concern. Both papers reported on a quasi-experimental evaluation of a quality improvement intervention. The titles, by-lines, and abstracts were similar and the methods sections almost identical. Moreover, entire paragraphs of the introduction and discussion sections were almost the same. Had an editorial crime been committed? And if so, was this a felony, a misdemeanor, or merely a technical breech akin to jaywalking? The first job of an editor in such circumstances is to establish the facts. Careful comparison of the two published papers showed that the same intervention was introduced (and evaluated) in two discrete inpatient settings: one involving housestaff, the other not. Furthermore, the patient populations, study designs, and analytic methods were distinct. What then, to make of a case where the same authors publish two papers using many of the same words to report on two similar but distinct interventions? What is self-plagiarism and how is it defined (if it exists at all)? To answer this question, we turned to the Committee on Publication Ethics (http://publicationethics.org) and the World Association of Medical Editors (www.wame.org), among other resources. Within hours of submitting the redacted case to the WAME listserv, we received a number of helpful replies, including one from Miguel Roig of St. Johns University. In prior writings, Roig has argued that self-plagiarism exists in four forms. Duplicate publication is publishing the same results in more than one article. Salami science is slicing up one large study into multiple small articles. Copyright infringement consists of using previously published material without permission of the copyright holder (which in JGIM’s case is the Society of General Internal Medicine). Text recycling is reusing phrases, sentences, or paragraphs found in previous work without appropriate attribution, including quotation marks. Each of these violations carries different moral weight. Duplicate publication is a clear ethical breech, in part because systematic reviews or meta-analyses may inadvertently count the same data twice, leading to mis-estimation of effects that could potentially harm patients. Salami science is undesirable but may be unavoidable without changes in the ways universities assess faculty research productivity (i.e., by counting papers). Copyright infringement is a legal issue, and interestingly, there have been few if any successful lawsuits involving this form of self-plagiarism. Finally, text recycling is a matter of degree. In our informal poll, many experts (including Roig) are fine with around 10% re-cycling of verbiage, some even arguing for the benefits of repeating complex methods verbatim. A few suggested limits of 15-20%, but none countenanced more than 30%. JGIM has no established limit for acceptable text re-cycling, nor do we currently plan to impose one. However, the JGIM editors wish to be clear about two things. <h4>Background</h4>Plagiarism is considered as serious research misconduct, together with data fabrication and falsification. However, little is known about biomedical researchers' views on plagiarism. Moreover, it has been argued - based on limited empirical evidence - that perceptions of plagiarism depend on cultural and other determinants. The authors explored, by means of an online survey among 46 reputable universities in Europe and China, how plagiarism is perceived by biomedical researchers in both regions.<h4>Methods</h4>We collected work e-mail addresses of biomedical researchers identified through the websites of 13 reputable universities in Europe and 33 reputable universities in China and invited them to participate in an online anonymous survey. Our questionnaire was designed to assess respondents' views about plagiarism by asking whether they considered specific practices as plagiarism. We analyzed if respondents in China and Europe responded differently, using logistic regression analysis with adjustments for demographic and other relevant factors.<h4>Results</h4>The authors obtained valid responses from 204 researchers based in China (response rate 2.1%) and 826 researchers based in Europe (response rate 5.6%). Copying text from someone else's publication without crediting the source, using idea(s) from someone else's publication without crediting the source and republishing one's own work in another language without crediting the source were considered as plagiarism by 98, 67 and 64%, respectively. About one-third of the respondents reported to have been unsure whether they had been plagiarizing. Overall, the pattern of responses was similar among respondents based in Europe and China. Nevertheless, for some items significant differences did occur in disadvantage of Chinese respondents.<h4>Conclusions</h4>Findings indicate that nearly all biomedical researchers understand (and disapprove of) the most obvious forms of plagiarism, but uncertainties and doubts were apparent for many aspects. And the minority of researchers who did not recognize some types of plagiarism as plagiarism was larger among China-based respondents than among Europe-based respondents. The authors conclude that biomedical researchers need clearer working definitions of plagiarism in order to deal with grey zones.

To further the progress of scientific research and expand the literature, authors and editors share a common goal of producing and reviewing innovative publications. However, as publication rates increase, so does the amount of detected plagiarism, including self-plagiarism. This concept, also referred to as text recycling, is defined as the repurposing of one's own previously published work in new publications without referencing the original source. There are advocates both for and against versions of text recycling, but without a universal protocol for authors and editors of what constitutes self-plagiarism, there is no strict standard among journals as to what is unethical. The advent of online text duplication detection software has been increasingly used by journals to assure that all published work is novel, but challenges remain. Converging on standardized guidelines would be beneficial with regard to text recycling and improving author education and the promotion of active communication between journals and authors during the submission process if confusion arises.

Published protocols have the potential to reduce bias in the conduct and reporting of systematic reviews (SR). When reporting the results of a completed SR, the question might arise whether text used in the protocol can also be used in the completed SR? Does this constitute text recycling, plagiarism, or even copyright infringement? In theory, no major changes to the protocol will be expected for the introduction and methods sections if the SR is completed in time. The benefits of maintaining the introduction and methods section of a protocol in the published SR are straightforward. Authors will require less time for writing up the completed SR. Potential benefits can also be expected for peer reviewers and editors. However, reusing text can be described as self-plagiarism. The question to be answered is whether this type of self-plagiarism is acceptable when copying text used previously (as would be the case when copying text from the protocol and pasting it into the subsequent completed SR)? The "traditional answer" to this question is "yes" because authors should not get credit for one piece of work for more than one time unless the work is cited appropriately. In contrast, we propose that in this context, it seems to be fully acceptable from a scientific and ethical perspective. As such, authors should not be accused of plagiarism in this case, but rather be encouraged to be efficient. However, legal issues need to be taken into consideration (e.g., copyright). We hope to stimulate a discussion on this topic among authors, readers, editors, and publishers.
