Background/Objectives: Toothbrushing is a recommended daily practice that helps sustain oral health. However, if performed improperly, it can lead to loss of tooth structure and injury to soft tissues. We explored this topic with an extensive literature search. Methods: A literature search was performed across textbooks and journals for original research and review articles in Scopus, PubMed, PubMed Central, and Cochrane databases, published between 1967 and 2024. Results: The search result yielded 118 articles that were suitable to include in this review. Toothpaste abrasivity plays a major role in combination with toothbrush forces. Therefore, maintaining forces between 2 and 3 N may be gentler on the tissue. Electric toothbrushes are safer. Toothpastes with low RDA values are also less abrasive. Active ingredients in whitening and desensitizing toothpaste can induce tooth wear. Remineralizing agents have the potential to manage the associated lesions. Conclusions: Cervical abrasions and gingival recession occur frequently due to oral hygiene measures. Standards in oral hygiene aid to match patient needs can prevent hard and soft tissue loss.

ABSTRACT Aim The aim of the present systematic review is to compare the effects of manual vs. powered toothbrushes on gingival lesions. Methods Comprehensive literature searches of PubMed, Embase, Scopus, and the Cochrane Library were conducted up to May 6, 2024, to identify randomized clinical trials examining the impact of manual and powered toothbrushes on gingival lesions. Two independent reviewers selected studies according to predefined criteria and extracted relevant data. Risk of bias was assessed using the RoB 2 tool, and certainty of evidence was evaluated using the GRADE approach. Sensitivity analyses were also performed to explore the influence of statistical model choice and study design on the pooled findings. This review was registered in PROSPERO (CRD42022368852). Results A total of 2978 records were identified, of which 10 studies met the inclusion criteria. The included studies comprised five parallel‐group randomized clinical trials and five crossover randomized clinical trials. Analysis of gingival recession showed no statistically significant difference between manual and powered toothbrushes at either 6‐ or 12‐month follow‐up (mean difference [MD]: −0.03; 95% confidence interval [95% CI]: −0.07 to 0.02). Studies evaluating gingival abrasion suggested a trend toward fewer abrasions with powered toothbrushes; however, the evidence was inconsistent and mostly based on qualitative findings. The certainty of evidence for both gingival recession and gingival abrasion was very low. Conclusions The available evidence does not demonstrate a clear difference between manual and powered toothbrushes regarding gingival recession progression. Although some studies suggested fewer gingival abrasions with powered toothbrushes, this finding should be interpreted with caution due to the very low certainty of evidence and the included studies presented important methodological limitations.

Although the total amount of tooth brushing over time has been thought to play a causal role in the development of gingival recession (Hirschfeld 1931 ), in the present 3‐year study no group differences emerged and neither the use of a power brush nor a manual brush was accompanied by increased gingival recession. On the contrary, there was evidence of reduced gingival recession with both brushes. It seemed likely that the procedures followed in this study resulted in improved tooth brushing behaviour and that this, in turn, reduced gingival recession. It has been suggested that with an improved technique, along with some slight reduction in gingival inflammation, there can be an element of creeping buccal attachment more usually seen after mucogingival surgery (Rajapakse et al. 2007 ). This offers a possible explanation for the reversal of gingival recession found in this study. Conclusions The findings of this study of the effects of toothbrushing on pre‐existing gingival recession are clear and compelling: Over a period of approximately 3 years of regular twice‐daily brushing, there was a significant and sustained reduction in gingival recession in both the group using a power toothbrush and the group using a manual brush. Over the same period, there was no difference in the amount of gingival recession between the two groups. Long‐term reductions in gingival recession may have been caused by sustained improvements in brushing technique due to the “Hawthorne Effect”. A regimen of 2 min. power or manual tooth brushing carried out daily, appears to have no adverse effects on gingival recession; it may even serve to improve the condition. Clinical Relevance.
