Are autistic children "behaviorists"? An examination of their mental-physical and appearance-reality distinctions.
This paper continues our earlier investigation of autistic children's deficit in attributing beliefs to others--in their "theory of mind." Three experiments are reported. The first tests the prediction that autistic children will fail to distinguish mental and physical entities. The second tests the prediction that they will also be unaware of the mental function of the brain. The third tests the prediction that they will be unable to take into account their own mental states. This latter prediction was tested using Appearance-Reality (A-R) tasks. All three predictions were supported. Deficits in these areas were not found among mentally handicapped or normal children of the same or lower mental and chronological age, suggesting that they may be autism-specific and independent of general developmental delay. It is argued that autistic children's failure to make A-R distinctions is consistent with Leslie's (1987) metarepresentation theory of autism.

The autistic child's theory of mind: a case of specific developmental delay.
In previous tests of the lowest level of a "theory of mind" (i.e. first-order belief attribution), 80% of autistic children were found to be impaired relative to a non-autistic mentally-handicapped control group. The present study examines the 20% of autistic children who have a theory of mind at the lowest level, and tests their ability to use a theory of mind at higher levels (i.e. second-order belief attribution). This autistic subgroup, in comparison to Down's Syndrome and normal control groups, was found to be severely impaired at the higher level. Autism is discussed as a possible case of specific developmental delay.

This systematic review focuses on the efficiency of using a Theory of Mind (ToM) curriculum in teaching children with Autism Spectrum Disorder (ASD) to apply social skills and decrease autistic characteristics. By analyzing eight well-designed studies using randomized controlled trials, quasi-experimental, and single subjects, the results indicate that ToM intervention may have an effect in social relationships, emotional comprehension, and thinking with reference to perspective. There is a gap in generalization and long-term retention of skills learned under the ToM in autistic children for short term courses. Individual interventions, as well as parental involvement, seem important in ensuring progress in the long-term. The review acknowledges the lack of longitudinal study to focus on durability, transferability, and the unique enabler approaches that can fix varying ToM dimensions and social deficits among children with autism. In general, the studies of ToM curricula have potential but require additional adjustment and investigation to be used widely and efficiently.

The systematic review implies that teaching Theory of Mind (ToM) curriculum to autistic children might be efficient in behavioral skills training and minimizing autistic symptoms. The research studies compared in the review involve diverse outc ome measures and designs, including but not restricted to randomized controlled studies, quasi -experimental designs and single-subject designs. The findings always show positive outcomes of ToM curriculum intervention, such as positive social skills and de crease in autistic traits. Relevant limitations of the studies involved in the systematic review including sample size, study design, and existing biases should be taken into consideration.
"Theory of mind" in Asperger's syndrome.
Two studies are reported in which the ability of people with Asperger's syndrome to understand problems of the type "Peter thinks that Jane thinks that ..." tested. The results showed that in contrast to younger or more handicapped autistic individuals, Asperger subjects were able to solve problems of the type just outlined, i.e. that they possessed a second-order theory of mind. When asked to explain their solutions however, they typically did not use mental state terms but did not differ in this respect either from non-handicapped or socially impaired, chronic schizophrenic controls. The implications of the results for current cognitive theories of autistic impairment are discussed.

Historically, autistic children were said to be delayed in developing a theory of mind, and the empathizing–systemizing theory proposed that while autistic people may have compassion (affective empathy) for others with similar autistic traits, they often have limited, though not necessarily absent, cognitive empathy. This may present as social naïvety, lower-than-average intuitive perception of the meaning or utility of body language, social reciprocity, or social expectations, including the habitus, social cues, and certain aspects of sarcasm, which to some degree may be influenced by co-occurring alexithymia. A 2019 meta-analysis found that alexithymia commonly co-occurs in autism (49.93%) and infrequently in neurotypicals (4.89%); the random effects risk ratio (6.50) was statistically significant (p<0.001).
Recent research has increasingly questioned these earlier interpretations, as the double empathy problem theory (2012) proposes that misunderstandings arise mutually between autistic and non-autistic people, rather than solely from autistic deficits in empathy or social cognition. This perspective has contributed to a growing recognition that autistic behavior and communication may reflect different, rather than deficient, social behavior and communication styles. Autistic interests and conversational styles are often characterized by a strong focus on specific topics, a phenomenon known as monotropism.
Solving belief problems develops as a skill in normal children during the preschool years. To understand this process of development, it is necessary to provide an analysis of the tasks used to test preschool 'theory of mind' skills. This analysis should allow us to relate the structure of a given task to the underlying cognitive mechanisms that the task engages. In two experiments, we find that 3-year-old children show a pattern of success and failure on belief tasks that is not consistent with 'conceptual deficit' accounts. Young children possess the concept, BELIEF, but have certain characteristic difficulties with correctly calculating the contents of beliefs. In childhood autism, by contrast, the mechanisms that in normal development bestow conceptual competence in this domain are impaired. In the first experiment, parallel task structures are used to show that 3-year-olds are no better at predicting behavior from a partially true belief than they are at predicting behavior from an entirely false belief. We develop specific proposals about task structural factors that either facilitate or hinder success in belief-content calculation. These proposals are supported in a second experiment. We compare two false-belief tasks, one of which has helpful structural factors, the other of which has hampering factors, with a third task which exemplifies a hampering task structure but without any theory of mind content.
