Objective. Coffee drinking is the main source of caffeine intake among adult population in the western world. It has been reported that low to moderate caffeine intake has beneficial effect on alertness and cognitive functions in healthy subjects. The aim of this study is to evaluate the impact of habitual coffee consumption on cognitive function in hemodialysis patients. Methods. In a cross-sectional study, 86 patients from a single-dialysis centre underwent assessment by the Montreal Cognitive Assessment tool and evaluation for symptoms of fatigue, mood, and sleep disorders by well-validated questionnaires. The habitual coffee use and the average daily caffeine intake were estimated by participants' response to a dietary questionnaire. Results. Sixty-seven subjects (78%) consumed black coffee daily, mostly in low to moderate dose. Cognitive impairment was found in three-quarters of tested patients. Normal mental performance was more often in habitual coffee users (25% versus 16%). Regular coffee drinkers achieved higher mean scores on all tested cognitive domains, but a significant positive correlation was found only for items that measure attention and concentration (P = 0.024). Conclusions. Moderate caffeine intake by habitual coffee consumption could have beneficial impact on cognitive function in hemodialysis patients due to selective enhancement of attention and vigilance. Caffeine (1,3,7-trimethylxanthine) is the most important and most extensively studied compound in coffee. The presumed stimulant effect of caffeine on physiological and mental states accounts for the habit-forming nature of coffee and is the main reason for the growing popularity of caffeinated beverages. The amount of caffeine available in prepared beverage is variable and principally depends on a proportion of the mix between two primary types of coffee beans, the preparation method, and the size of the coffee cup. It is estimated that ingestion of a single cup of coffee provides doses from 0.4 to 2.5 mg of caffeine per kilogram. Since none of the responders reported the regular intake of other caffeinated beverages or food besides black coffee, we equated its using with the total amount of caffeine. Those study subjects drinking at least one cup of coffee per day were further asked to classify their usual method of preparing coffee and the size of serving cup. We assessed the total intake of caffeine by summing the caffeine content for a specific amount, depending on the preparation method and cup size, multiplied by the frequency of its use. The estimated caffeine content per single cup was 120 mg, based on data about type and an average serving size of brewed black coffee consumed in Serbia. 2.6. P values less than .05 were considered statistically significant. 3. Results More than three-quarters (78%) of the eighty-six eligible study participants consumed black coffee on a regular basis. In the group of sixty-seven regular coffee drinkers, low use (1 cup/day) was reported by 36% ( n = 24), moderate use (2 cups/day) by 40% ( n = 27), and intake of more than 2 cups a day by 24% ( n = 16) of the examined patients. Only two patients (3%) have reported heavy coffee use (≥5 cups/day) on a regular basis. The estimated adjusted amount of consumed caffeine, which almost exclusively came through ingestion of separate portions of black coffee, was 3.4 mg/kg/day (250 mg). There was no statistical significant difference in the habitual coffee drinking and total amount of ingested caffeine comparing fifteen patients who met exclusion criteria and the patients included in the study. More than 90% of habitual users in our study reported drinking coffee predominantly in the morning and early afternoon, which is followed by overnight abstinence. There were no significant differences in demographics, clinical characteristics, lifestyle factors, and selected medical comorbidities between persons who drink coffee daily and noncoffee users. Prevalence of subjects with less than 8 years of education did not differ substantially between noncoffee and regular coffee drinkers. Attention and concentration help us to select and focus on what is important by modifying neural activity in specific cortical areas, thus augmenting the cerebral response to particular perceptual stimuli [ 31 ]. Caffeine influences stimulus processing and enhances vigilance by increasing cortical activation and the rate at which information accumulates. Thus, the regular caffeine intake by coffee drinking may effectively modulate the encoding and long term retrieval of memories by enhancing sustained attention [ 32 , 33 ]. The strengths of this study include the well-defined single center subject sample, a comprehensive estimation of cognitive function, and standardized evaluations of mood state, sleep disturbance, and level of fatigue. In addition, all MoCA evaluations were administered by a single physician. Finally, it should be noted that, even though in public perception and our paper the terms coffee and caffeine are interchangeable, the biologic effects of coffee cannot be reduced to the isolated effects of caffeine that it contains [ 38 ]. 5.

Human cognitive impairment associated with sleep loss, circadian misalignment and work overload is a major concern in any high stress occupation but has potentially catastrophic consequences during spaceflight human robotic interactions. Two safe, wake-promoting countermeasures, caffeine and blue-enriched white light have been studied on Earth and are available on the International Space Station. We therefore conducted a randomized, placebo-controlled, cross-over trial examining the impact of regularly timed low-dose caffeine (0.3 mg per kg per h) and moderate illuminance blue-enriched white light (~90 lux, ~88 melEDI lux, 6300 K) as countermeasures, separately and combined, in a multi-night simulation of sleep-wake shifts experienced during spaceflight among 16 participants (7 F, ages 26–55). We find that chronic administration of low-dose caffeine improves subjective and objective correlates of alertness and performance during an overnight work schedule involving chronic sleep loss and circadian misalignment, although we also find that caffeine disrupts subsequent sleep. We further find that 90 lux of blue-enriched light moderately reduces electroencephalogram (EEG) power in the theta and delta regions, which are associated with sleepiness. These findings support the use of low-dose caffeine and potentially blue-enriched white light to enhance alertness and performance among astronauts and shiftworking populations. On Earth, short sleep duration, circadian misalignment and long work hours have repeatedly been linked to cognitive impairment in other professions with highly trained and highly motivated personnel 8 , including an increase in attentional failures 9 , actual and near-miss medical errors 10 and fatigue-related motor vehicle crashes 11 among medical residents, and poor health outcomes in shiftworkers 12 – 15 . While measures should be taken to provide crewmembers with stable sleep schedules in an optimized environment, direct, safe, wake-promoting countermeasures are often needed to minimize the safety risk due to sleepiness when sleep and work patterns are not optimal. Two safe, wake-promoting countermeasures are feasible for use on future space vehicles and are currently available on the International Space Station (ISS). Caffeine, the most widely used performance-enhancing drug, is provided during spaceflight in the form of pills and liquid coffee although its use is currently unregulated. Numerous studies have confirmed the utility of caffeine as a countermeasure against the effects of sleepiness on certain types of neurobehavioral performance 16 – 20 including for up to 28 hours awake when administered chronically in low doses (0.3 mg per kg per h) 21 . Another stimulant that is available to the ISS crew is light. Light is both an acute stimulant 22 , 23 and the most powerful time cue for resetting the circadian pacemaker and the rhythms it controls, including the sleep/wake cycle 24 . These ‘non-visual’ effects of light are primarily mediated by a novel non-rod, non-cone photoreceptor in the eye that is most sensitive to short-wavelength light ( λ max ~ 480 nm) 25 – 28 . Blue and blue-enriched light have been used as countermeasures to enhance alertness and performance in laboratory 25 , 26 , 29 , 30 and field 31 – 33 studies. The combination of caffeine and blue-enriched light exhibited the largest effect sizes for some outcomes, suggesting a potential positive effect of using both stimulants together. Caffeine is the most widely used wake-promoting drug worldwide. Regularly timed administration of low-dose caffeine (equivalent to only 21 mg per h in a 70 kg person, or less than half a standard soda or tea) elicited sustained high performance on the PVT during a repeated ‘slam shift’ involving a sudden transition to night work following a week of chronic sleep restriction of six hours. It also improved performance on the DSST and on subjective ratings of sleepiness. Sleep and circadian-rhythm disorders were exclusionary. Participants were instructed to maintain a self-selected, regular sleep-wake schedule of 8 hours of sleep and 16 hours of wake for at least two weeks, followed by a week of 6 hours sleep per night, timed to end at their habitual wake time, prior to laboratory admission. The sleep times were verified by calls to a time- and date-stamped voicemail and continuous actigraphy monitoring (Actiwatch-L; Minimitter, Inc.). After 9 hours of wakefulness, subjects went to bed at their habitual sleep time and were allowed to sleep for 6 hours. The IV was removed upon waking on wake period 6 (day 4) and another cycle of slam-shifting (WP7, WP8) and robotics testing (WP8) was initiated, identical to the first one described above, which was repeated two more times throughout the protocol (WP12, WP 16) before the subjects were discharged on the evening of day 12 after a final 8-hour recovery sleep. We chose to use three, rather than all six, possible countermeasure orders in an attempt to reduce between-subject sleep-debt effects that we expected to be large, and given the relatively small number of participants available. The original design included a single fixed order for all participants but upon reflection, this was modified before study start to address the limitations of that approach. Low-dose caffeine (0.3 mg per kg body weight) or placebo was administered orally in pill form by a nurse every hour starting at wake time until 2 hours prior to bedtime during countermeasure testing (WP4, WP8, WP12, WP16).

Coffee, cardiovascular and behavioral effects current research trends. This review attempts to summarize cardiovascular and behavioral research on coffee over the last five years. Interest in these areas has increased relative to earlier periods, and new aspects have been considered. Acute cardiovascular effects induced by giving average total daily doses as a single dose to non-users or abstaining users have been compared between normotensives and hypertensives, between habitual users and non-users, and with and without additional mental stress. Implications for daily use were studied semichronically. Great interest has also been devoted to the putative atherogenic effects and to epidemiological associations with cardiovascular disease. Behavioral research has continued, in part, the tradition of assessing effects on performance and mood in random samples, in users and non-users, and in different personality types. Objective measures of sleepiness were increasingly used to assess the maintenance of wakefulness and the antagonism of physiologically or pharmacologically induced sleepiness.
